Crossover House Program – Crossover House provides outreach, engagement, mental health and transition services for individuals who are on the streets and in shelters who may experience a severe mental illness or co-occurring substance abuse disorders. The focus is on the early stages of treatment through outreach and engagement of individuals who are not currently or have never been involved with the mental health system. Crossover House is generally the gateway into homeless and housing services within ACMHS. Services are provided in shelters and in the community where homeless persons congregate. Intensive case management services are provided to assist individuals to make the choice to become involved in formal treatment services and break the cycle of homelessness.
Pathways to Recovery IV – Pathways is a street outreach, engagement and intensive case management program targeting the chronic public inebriate and those with co-occurring severe mental illness. Staff work to engage individuals into treatment and recovery from substance abuse with the goal of successful placement in long term permanent housing with supports.
Transitional Program – The Transitional Housing Program is for persons who are homeless, mentally ill and in transition from the streets and shelters into more permanent and independent housing. The program is designed to provide comprehensive behavioral health, social and interpersonal skill development, life skills development and other rehabilitative services designed to assist clients in acquiring the skills necessary for transition into permanent housing. The program can serve 20 individuals who are referred by the Crossover House Program.
Permanent Housing Program – Permanent Housing focuses on providing intensive support services and permanent housing to persons who are homeless and mentally ill. Many experience co-existing disorders in addition to severe mental illness. Individuals are referred to the Permanent Housing Program by other clinical teams within ACMHS or at intake, as appropriate. Clients must meet Federal definition of homelessness and inability to maintain residential stability with standard support services for sustained periods of time. The purpose of the program is to provide long-term permanent housing and the supports necessary to maintain that housing. Clients are primarily referred by the Crossover House Program.
Shelter Plus Care Program – The Shelter Plus Care Housing Program provides rental assistance to persons who are homeless and experience a severe mental illness. Consumers can choose from scattered site apartments in the private rental market. Behavioral health services are provided by ACMHS staff and referrals are made to other providers in the community. Clients must meet Federal definition of homelessness and inability to maintain residential stability with standard support services for sustained periods of time. The program can service up to 28 individuals and families at any one time depending on level of rental assistance. Referrals are made by other programs within ACMHS.
Bridge Home Program – The Bridge Home Program serves clients who have a high utilization of Alaska Psychiatric Institute and/or Department of Corrections. Bridge Home clients are referred directly from these institutions, the Division of Behavioral Health, and within Anchorage Community Mental Health Services based on their utilization rates. In addition, they must be eligible for Alaska Housing Finance Corporation housing assistance and have the ability to live in an independent apartment with supports. The program works to permanently house clients with extensive and ongoing supportive services. The primary goal is to decrease the clients time spent in API and DOC through access to recovery services. The program can serve approximately 50 clients a year.
